Three Ways To Register

Thank you for your interest in classes at BAYarts. Please complete the registration form and health release form (for students under 18).  Once your forms have been completed, please submit the form in one of the following three options.

• Fax the form to us at 440.871.0452
• Mail the form
• Drop off the form in person
